Orthodontia coverage (12 month waiting period and lifetime maximum of $1,500)
VISION COVERAGE HIGHLIGHTS
Basic eye exam, eye refraction, single lenses, bifocal lenses and progressive lenses (no waiting period and one per year)
Frames and lenses such as eyeglass frames, contact lenses, anti-reflective lenses, polycarbonate lenses and contact lens fitting fee (6 month waiting period for eyeglass frames, no waiting period for others)
HEARING COVERAGE HIGHLIGHTS
Hearing exam, hearing aid and necessary repairs or supplies (12 month waiting period)
